Motor yacht SKAT Is an impressive 70,70-metre luxury superyacht built by the well-known and respected German shipyard Lurssen and launched in 2002. Superyacht SKAT was constructed under the Project name 9906 based on the design by Espen Oeino International with interiors by the Italian designer Marco Zanini.

SKAT features a steel hull and an aluminium superstructure, with a gross tonnage of 1998 tonnes. SKAT's controversial design by Espen Oeino is often referred to as militaristic, offering a truly unique style to her owner.

Some of the main features and amenities of SKAT include a gymnasium, two superyacht tenders and a motorcycle. There is also a helicopter that accompanies SKAT that has been painted in matching grey colour.

Luxurious and sumptuous accommodation was created in five beautiful cabins, sleeping up to ten guests in total. SKAT yacht has a crew of around 20 members, keeping her well-maintained and offering impeccable service to her owner and guests.

SKAT Specifications

The yacht has been fitted with the latest in stabilisation systems, reducing roll motion effect, resulting in a smoother ride. It also has 'at anchor' stabilisers offering additional comfort to her guests.

Mega yacht SKAT is powered by two MTU engines (16V 4000 M70) of 2,717 hp (2,026KW) each, powering her up to a maximum speed of 17 knots and allowing her to cruise at a speed of 15 knots.

Lürssen’s 90m expedition yacht NORN delivered

Following her launch in February earlier this year, the highly secretive 90m (295 ft) superyacht NORN , formerly known as Project 1601, has been delivered by iconic German builder Lürssen and has since left the facilities to embark on her maiden voyage.

The specifications and amenities for superyacht NORN have been kept a closely guarded secret since her construction began in 2018. However, we know her distinct, geometrical exterior was helmed by Norwegian designer and naval architect Espen Oeino whilst her yet-to-be-revealed interiors are the works of Dölker+Voges Design and are expected to mirror the yacht’s exterior design.

The brief required a superyacht that excels in design and engineering whilst also catering to maximum comfort onboard. Described by the shipyard as a project with “striking design" and "extraordinary features”, superyacht NORN has a utilitarian character, with an oblique reverse bow, narrow rectangular windows, planar surfaces and angular corners.

Built for a repeat owner, superyacht NORN is a replacement vessel for the iconic 70m (232ft) yacht SKAT , which was delivered in 2002.

Sharing similar design cues, we can expect that brand new superyacht NORN may follow an alike minimalist interior approach with a variety of light, bright and airy social spaces onboard. There is also expected to be a signature panoramic sky lounge, an additional area onboard where guests can soak up the superyacht lifestyle.

The brief required a superyacht that excels in design and engineering to provide seagoing comfort for the family.

As a custom-built 3,600 GT explorer yacht , NORN is guaranteed to have a helipad, closing the bridge between land and sea for guests onboard. She also features zero-speed stabilizers, an exterior cinema and a liftable pool floor with dancefloor illumination to allow a unique and memorable cruising experience for her guests. Electronic anchoring in sensitive and remote areas is made easy thanks to the use of a dynamic positioning system, ensuring that the seabed is protected regardless of water depth.

Norn was one of two yachts launched by the German shipyard in February following the grand reveal of the 145m (475ft) Project Luminance , which is the eighth largest yacht to be built by Lürssen and has just successfully finished her sea trials last week.

With over 10 projects in-build currently, Lürssen is expected to deliver the 122m (400ft) superyacht PROJECT JAG later this year whilst the 114 (378ft) superyacht PROJECT COSMOS will be completed in 2024.

For the first time Rosatom Fuel Division supplied fresh nuclear fuel to the world’s only floating nuclear cogeneration plant in the Arctic

The fuel was supplied to the northernmost town of Russia along the Northern Sea Route.

The first in the history of the power plant refueling, that is, the replacement of spent nuclear fuel with fresh one, is planned to begin before 2024. The manufacturer of nuclear fuel for all Russian nuclear icebreakers, as well as the Akademik Lomonosov FNPP, is Machinery Manufacturing Plant, Joint-Stock Company (MSZ JSC), a company of Rosatom Fuel Company TVEL that is based in Elektrostal, Moscow Region.

The FNPP includes two KLT-40S reactors of the icebreaking type. Unlike convenient ground-based large reactors (that require partial replacement of fuel rods once every 12-18 months), in the case of these reactors, the refueling takes place once every few years and includes unloading of the entire reactor core and loading of fresh fuel into the reactor.

The cores of KLT-40 reactors of the Akademik Lomonosov floating power unit have a number of advantages compared to the reference ones: a cassette core was used for the first time in the history of the unit, which made it possible to increase the fuel energy resource to 3-3.5 years between refuelings, and also reduce the fuel component of the electricity cost by one and a half times. The FNPP operating experience formed the basis for the designs of reactors for nuclear icebreakers of the newest series 22220. Three such icebreakers have been launched by now.

For the first time the power units of the Akademik Lomonosov floating nuclear power plant were connected to the grid in December 2019, and put into commercial operation in May 2020. The supply of nuclear fuel from Elektrostal to Pevek and its loading into the second reactor is planned for 2024. The total power of the Akademik Lomonosov FNPP, supplied to the coastal grid of Pevek without thermal energy consumption on shore, is about 76 MW, being about 44 MW in the maximum thermal power supply mode. The FNPP generated 194 million kWh according to the results of 2023. The population of Pevek is just a little more than 4 thousand, while the FNPP has a potential for supplying electricity to a city with a population of up to 100 thousand people. After the FNPP commissioning two goals were achieved. These include first of all the replacement of the retiring capacities of the Bilibino NPP, which has been operating since 1974, as well as the Chaunskaya TPP, which has already been operating for more than 70 years. Secondly, energy is supplied to the main mining companies in western Chukotka in the Chaun-Bilibino energy hub a large ore and metal cluster, including gold mining companies and projects related to the development of the Baimsk ore zone. In September 2023, a 110 kilovolt power transmission line with a length of 490 kilometers was put into operation, connecting the towns of Pevek and Bilibino. The line increased the reliability of energy supply from the FNPP to both Bilibino consumers and mining companies, the largest of which is the Baimsky GOK. The comprehensive development of the Russian Arctic is a national strategic priority. To increase the NSR traffic is of paramount importance for accomplishment of the tasks set in the field of cargo shipping. This logistics corridor is being developed due regular freight voyages, construction of new nuclear-powered icebreakers and modernization of the relevant infrastructure. Rosatom companies are actively involved in this work. Rosatom Fuel Company TVEL (Rosatom Fuel Division) includes companies fabricating nuclear fuel, converting and enriching uranium, manufacturing gas centrifuges, conducting researches and producing designs. As the only nuclear fuel supplier to Russian NPPs, TVEL supplies fuel for a total of 75 power reactors in 15 countries, for research reactors in nine countries, as well as for propulsion reactors of the Russian nuclear fleet. Every sixth power reactor in the world runs on TVEL fuel. Rosatom Fuel Division is the world’s largest producer of enriched uranium and the leader on the global stable isotope market. The Fuel Division is actively developing new businesses in chemistry, metallurgy, energy storage technologies, 3D printing, digital products, and decommissioning of nuclear facilities. TVEL also includes Rosatom integrators for additive technologies and electricity storage systems. Rosenergoatom, Joint-Stock Company is part of Rosatom Electric Power Division and one of the largest companies in the industry acting as an operator of nuclear power plants. It includes, as its branches, 11 operating NPPs, including the FNPP, the Scientific and Technical Center for Emergency Operations at NPPs, Design and Engineering as well as Technological companies. In total, 37 power units with a total installed capacity of over 29.5 GW are in operation at 11 nuclear power plants in Russia. Machinery Manufacturing Plant, Joint-Stock Company (MSZ JSC, Elektrostal) is one of the world’s largest manufacturers of fuel for nuclear power plants. The company produces fuel assemblies for VVER-440, VVER-1000, RBMK-1000, BN-600,800, VK-50, EGP-6; powders and fuel pellets intended for supply to foreign customers. It also produces nuclear fuel for research reactors. The plant belongs to the TVEL Fuel Company of Rosatom.

Innocent Heart Murmurs

What is a heart murmur.

A heart murmur is an extra sound heard while listening to the heart. The noise is caused by blood flowing through a normal heart. This is called an “innocent” murmur.

Is this common in children?

Yes. Innocent heart murmurs can be heard in 50-75% of healthy children.

What causes it?

There is not one single reason for the cause of innocent murmurs. It is the sound of normal blood flow in large blood vessels. There may also be extra strands of muscle in the pumping chamber of the heart. There may be many causes of an “innocent” murmur.

Will this murmur go away? If it doesn't, should I be worried?

Innocent murmurs may become louder when you are sick. There may be times when your doctor cannot hear the murmur. If the murmur does not go away, there is no reason for concern. “Innocent" murmurs do not affect how well the heart works.

How can you tell which murmurs are innocent?

Your heart doctor (cardiologist) will take a complete health history and medical exam. The doctor will decide if you need further testing to make the diagnosis.

Are there any special limits and do I need a follow-up visit?

No. From a heart standpoint, you can do your normal routine. You do not need antibiotics when going to the dentist or having surgery. You will not need to return to the clinic. The heart doctor will give you any further instructions if needed.

Heart Murmurs: The Difference Between Innocent And Abnormal

Sometimes, literally listening to your body can make doctors aware of potentially fatal health conditions.

Heart murmurs are sounds made by blood as it passes through openings in the heart, such as valves. Your doctor can hear these murmurs while listening to your heart during a physical exam.

Murmurs are sometimes associated with heart problems. And other times they may be present at birth and can go away with time. Tiberio Frisoli, M.D. , a cardiologist at Henry Ford Health, clarifies the difference between a benign murmur and a murmur that could lead to the diagnosis of a serious heart condition.

How are heart murmurs identified?

“Heart murmurs are not a symptom or a diagnosis but rather a physical exam finding, and they can be the first clue that leads to the diagnosis of a heart disease condition,” says Dr. Frisoli. “While a bad murmur can be a sign of a bad heart condition, not having a murmur does not exclude the presence of a heart condition. In other words, a lot of heart disease conditions do not cause murmurs.”

Your doctor will use an echocardiogram – an ultrasound of the heart – to determine if a murmur is innocent or abnormal. These murmurs are identified by several key characteristics including:

Intensity: how loud the murmur is
Pitch: whether it produces a high or low frequency
Quality: an increase or decrease in the sound of the murmur
Location: where in the chest the murmur is heard
Timing: at what point in the cardiac cycle the murmur is heart

What is the difference between an innocent and an abnormal murmur?

Innocent murmurs.

“An innocent murmur is simply a noise that can sometimes occur as blood passes through a normal heart,” says Dr. Frisoli. “In fact, there are no symptoms associated with them.”

Often, children are born with heart murmurs that go away once they’re adults. Similarly, some pregnant women develop heart murmurs because they have more blood volume passing through the heart valves. These murmurs usually go away after pregnancy.

Abnormal murmurs

“Abnormal heart murmurs can be caused by a large variety of valve conditions,” say Dr. Frisoli. “They can occur if you have heart disease or holes in your heart – whether from birth or acquired, such as after a heart attack.”

Murmurs caused by heart disease are called pathologic murmurs. They occur when your blood travels through a leaky or narrowed heart valve. With the heart conditions associated with this type of murmur, you might experience symptoms such as:

Shortness of breath
Leg swelling
Lightheadedness
Chest pressure (especially when active)

These more serious murmurs usually do not go away unless the problem that causes them is treated. If you have gone through treatment to replace or repair a heart valve, your murmur may change sound or go away completely. Likewise, murmurs can get worse if a condition goes untreated or becomes more serious.

Your heart is unique, and some heart murmurs can change over time. Because of this, it is important for your doctor to listen to your heart every time you go in for a physical exam. If you have a heart murmur, your doctor may order an echocardiogram, refer you to a heart specialist and make sure you schedule annual visits to monitor your heart health.

What's that sound? A heart murmur can be innocent or serious

You may have heard the term "heart murmur." A murmur is an extra heart sound that can be heard by a stethoscope. Sometimes, the murmur sounds like a humming, which can be faint or loud. It might be temporary or persistent. Heart murmurs may be present at birth or develop later in life during pregnancy, phases of rapid growth like adolescence or from a fever or anemia.

The murmur may disappear as quickly as it comes if it has a temporary cause. However, a murmur also could be persistent and loud, easily heard and sound like a churning mill wheel or cooing seagull. This might indicate a serious heart problem.

What causes a heart murmur?

Multiple factors can cause a murmur. It could be a heart valve problem or a hole in the heart.

The valves in your heart act as doors between the chambers, or rooms, of the heart. In the case of a murmur, a valve may be tight or leaky. When heart valves are narrow, this is called stenosis. A murmur also could be from a leaky valve , called regurgitation.

A murmur also may occur from high blood flow in people with a fever or with low red blood cells, called anemia.

Some people have a family history of heart murmur and heart disease. Some are born with a congenital condition causing a murmur. Others have had a recent severe infection or illness that could damage a heart valve and need immediate medical attention.

What are the symptoms of a heart murmur?

Innocent or harmless heart murmurs don't typically cause symptoms, and most heart murmurs aren't serious.

Symptoms of a serious or worrisome heart murmur depend on the cause and require evaluation by a healthcare professional.

These heart murmur symptoms may include:

Blue or gray lips or fingernails
Lingering cough
Sudden weight gain
Swollen liver or neck veins
Worsening fatigue

How is a heart murmur diagnosed?

It’s common for a heart murmur to be detected during a physical exam being given for another reason. If a healthcare professional hears a murmur, you'll be asked questions about your personal and family history that could indicate a reason for the murmur.

Several criteria are used to determine if a murmur is innocent or worrisome, including:

Volume The loudness of the heart murmur is evaluated on a scale from 1 to 6. The loudest heart murmur is a 6.
Location The location of the murmur in the heart will be identified, along with whether the sound spreads to the neck or back.
Pitch The heart murmur may be high-, medium- or low-pitched.
Timing A heart murmur that occurs when blood leaves the heart is a systolic murmur. A murmur that occurs when the heart fills with blood is called a diastolic murmur. A murmur also may be heard throughout the heartbeat. When the murmur is heard as blood passes through the heart, it may be a sign of a larger heart problem.

You will need to undergo testing to determine the cause of the heart murmur. Your healthcare professional likely will order an ultrasound picture of your heart, called an echocardiogram or echo, to show detailed images of your heart's valves, chambers, structure and function.

Once the cause of a heart murmur has been found, some people will need repeated evaluation over the years. Others may need surgery to treat the cause of the murmur, especially if it's to adjust a tight or leaky valve, or to close a hole in the heart.

In some situations, people may need to take antibiotics before going to a dentist to protect the heart valves from potential infection. This infection, called endocarditis , can be spread via the bloodstream during dental work.

Innocent heart murmurs

An innocent murmur can occur when blood flows more rapidly than normal through the heart. Conditions that may cause rapid blood flow through your heart, resulting in an innocent heart murmur, include:

Physical activity or exercise
Not having enough healthy red blood cells to carry adequate oxygen to your body tissues (anemia)
An excessive amount of thyroid hormone in your body (hyperthyroidism)
Phases of rapid growth, such as adolescence

Innocent heart murmurs may disappear over time, or they may last your entire life without ever causing further health problems.

Abnormal heart murmurs

The most common cause of abnormal murmurs in children is when babies are born with structural problems of the heart (congenital heart defects).

Common congenital defects that cause heart murmurs include:

Holes in the heart or cardiac shunts. Known as septal defects, holes in the heart may or may not be serious, depending on the size of the hole and its location. Cardiac shunts occur when there's an abnormal blood flow between the heart chambers or blood vessels, which may lead to a heart murmur.
Heart valve abnormalities. Congenital heart valve abnormalities are present at birth, but sometimes aren't discovered until much later in life. Examples include valves that don't allow enough blood through them (stenosis) or those that don't close properly and leak (regurgitation).

Other causes of abnormal heart murmurs include infections and conditions that damage the structures of the heart and are more common in older children or adults. For example:

Valve calcification. This hardening or thickening of valves, as in mitral stenosis or aortic valve stenosis, can occur as you age. Valves may become narrowed (stenotic), making it harder for blood to flow through your heart, resulting in murmurs.
Endocarditis. This infection of the inner lining of your heart and valves typically occurs when bacteria or other germs from another part of your body, such as your mouth, spread through your bloodstream and lodge in your heart.Left untreated, endocarditis can damage or destroy your heart valves. This condition usually occurs in people who already have heart valve abnormalities.
Rheumatic fever. Although now rare in the United States, rheumatic fever is a serious condition that can occur when you don't receive prompt or complete treatment for a strep throat infection. It can permanently affect the heart valves and interfere with normal blood flow through your heart.

Risk factors

There are risk factors that increase your chances of developing a heart murmur, including:

Family history of a heart defect. If blood relatives have had a heart defect, that increases the likelihood you or your child may also have a heart defect and heart murmur.
Certain medical conditions. Uncontrolled high blood pressure (hypertension), hyperthyroidism, an infection of the lining of the heart (endocarditis), high blood pressure in the lungs (pulmonary hypertension), carcinoid syndrome, hypereosinophilic syndrome, systemic lupus erythematosus, rheumatoid arthritis, a weakened heart muscle or a history of rheumatic fever, can increase your risk of a heart murmur later in life.

Factors that increase your baby's risk of developing a heart murmur include:

Illnesses during pregnancy. Having some conditions during pregnancy, such as uncontrolled diabetes or a rubella infection, increases your baby's risk of developing heart defects and a heart murmur.
Taking certain medications or illegal drugs during pregnancy. Use of certain medications, alcohol or drugs can harm a developing baby, leading to heart defects.

What is a heart murmur?

A heart murmur is an abnormal noise between heartbeats. It can be a blowing, whooshing, or swishing sound. When your doctor listens to your heartbeat with a stethoscope, they are listening to the sounds your blood makes as it flows through your heart. A heart murmur is when the blood makes an extra sound as it moves through. A heart murmur can be a sign of a heart problem. It can also be caused by a minor issue or a normal increase in blood flow in the heart.

Symptoms of a heart murmur

A heart murmur can be a symptom of another problem. The sound is the only way to know if you or your child has one. If a murmur is caused by a heart problem, you may have other symptoms. These could include:

Dizziness or fainting
Bluish skin

Many different things can cause heart murmurs. In many cases, the heart murmur is just the sound of blood flowing through a normal, healthy heart. This is called an “innocent” or “functional” murmur. These noises are commonly heard in children because their hearts are very close to their chest walls. Many innocent murmurs become hard to hear as children grow older. Most go away on their own. Pregnant women also sometimes have innocent heart murmurs because the body makes extra blood during pregnancy.

Other heart murmurs are caused by problems with the heart. Heart problems that can cause a murmur include:

Defects in the septum. The septum is the wall between the left and right chambers of the heart. Sometimes a hole in the septum can let blood flow through it. This blood flow causes a murmur.
Abnormalities with valves. The valves in the heart let the blood flow from the upper chambers to the lower chambers. Sometimes there are problems with these valves. The valves could be too thick, too narrow, or too “floppy.” These problems can affect the flow of blood, which can cause a murmur.
Disorders with the heart muscle. These problems (called cardiomyopathy) can make the muscle of the heart too thick or too weak. This affects the heart’s ability to pump blood and can cause extra noises.

You are at higher risk of developing a heart murmur if you have a family history of a heart defect or certain medical conditions. These could include high blood pressure, hyperthyroidism, or endocarditis (infection in the lining of the heart).

Your baby is at higher risk of developing a heart murmur if you used certain medicines, alcohol, or drugs during pregnancy. Other pregnancy conditions can increase the risk, such as uncontrolled diabetes or a rubella infection.

Usually, your doctor will find a murmur during a regular exam when listening to your heart with a stethoscope. Once they hear the murmur, they will listen to the loudness, the location, and the timing of it. This will help them determine if it is an innocent murmur or if it could be something more serious.

If your doctor suspects a heart problem, they may refer you to a cardiologist. This is a doctor who specializes in the heart. Your doctor may choose to do some testing. These tests could include a chest X-ray, an electrocardiogram (EKG or ECG), or an echocardiogram (sometimes called an “echo,” which is an ultrasound of the heart). These tests will help them see if there are problems with your heart rhythm, your heart structure, or how your heart is working.

Can a heart murmur be prevented or avoided?

There is nothing you can do to prevent heart murmurs caused by genetics. Avoiding drugs and alcohol and receiving treatment for infections during pregnancy can prevent some heart murmurs in children.

Heart murmur treatment

The treatment for a heart murmur depends on what is causing the murmur. An innocent heart murmur doesn’t need any treatment. If a heart problem or other condition is causing the murmur, that problem will need to be treated. These types of conditions are usually treated with medicine. Severe cases could require surgery.

What if my child has an innocent heart murmur?

An innocent heart murmur does not pose a health threat. If your child has an innocent heart murmur, they can run, jump, and play, with no limits on activity. Your child doesn’t need to take any medicine or be careful in any special way.

Living with a heart murmur

If you or your child has an innocent heart murmur, you can live a completely normal life. It will not cause you any problems and is not a sign of an issue with your heart. If you have a murmur along with any of the following symptoms, see your doctor:

You are very tired
You have trouble exercising or being physically active
You have chest pain

If your child has a heart murmur and any of these symptoms, call your doctor:

Rapid breathing
Difficulty feeding
Blue tint to the lips

Innocent Heart Murmur

Heart murmur is the most common reason for a referral to a pediatric cardiologist. Virtually all children have a heart murmur during their childhood. Less than 1% of murmurs are pathological in children. Innocent/functional heart murmur is the most common type of heart murmur. There are multiple theories proposed to identify etiology of innocent heart murmur with varying consensus, but everybody agrees that innocent heart murmur does not carry any morbidity or mortality risk. Even today, heart murmur is associated with high physician uncertainty and parental anxiety. Extensive cardiac evaluation for such a benign finding is also associated with high health care utilization and cost. This article attempts to review this long-known finding which continues to remain a diagnostic challenge.

Introduction and background

Cardiac murmurs have remained a “hot topic” in cardiac evaluation since the invention of the stethoscope by Rene Laennec in the early 17th century [ 1 ]. Heart murmur continues to remain the most common reason for referral to pediatric cardiologist [ 2 ]. Less than 1% of pediatric heart murmurs are associated with congenital heart disease, and most of the heart murmurs are innocent in nature [ 3 ]. Terminologies like “innocent”, “physiologic”, “benign”, “normal” or “functional” are frequently used but calling it “innocent” clearly conveys the non-pathological nature of this finding to the parents and patients.

Murmur is a sound produced by vibrations caused by the flow of blood through the heart [ 4 ]. Several different mechanisms are proposed regarding the development of any murmur such as turbulence from the blood flow, cardiac vibrations, Bernoulli effect, eddy current, etc. Dr. George Frederic Still, in the year 1909, first described “musical murmur” in pediatric patients and deemed it as innocent [ 5 ].

Types of innocent heart murmurs

Innocent heart murmurs in children can be categorized under four distinct groups: Still’s vibratory murmur, pulmonary flow murmur, supraclavicular systemic flow murmur, and venous hums. Many high output states like anemia, fever, arteriovenous malformation, etc. can also result in heart murmur [ 5 ].

Still’s murmur: McKusick et al. very appropriately suggested the musical instrument, Aeolian Island harp, as an analogue for Still’s murmur [ 6 ]. Still’s murmur is a brief, vibratory quality, grade 1-3, midsystolic, and low-pitched murmur. This is best heard with the bell of a stethoscope. This murmur is heard at the left lower sternal border and occasionally radiates to the cardiac apex. Typical age group for this murmur is three years to early adolescence, but it can be present at any age. Because the innocent murmurs are produced by normal flow dynamics, any alteration in the flow with a change in patient position will change the murmur characteristics. This murmur is the loudest in the supine position and it diminishes in intensity when a child is made to sit or stand up, as these positions diminish the venous return to the heart. This could be a valuable tool while examining Still’s murmur in the clinic.

Some of the proposed mechanisms for the development of Still’s murmur are the presence of “fibrous bands” or “false tendon” in the left ventricle [ 7 - 8 ], smaller aortic size causing increase in the flow velocity [ 9 ], insertion of tricuspid valve chordae into the right ventricular outflow tract [ 5 ], increased left ventricular output with relative bradycardia [ 10 ], vibrations from cardiac structures [ 10 ], and lower aortic arterial elastance with a higher left ventricular contractility [ 11 ]. There is no clear consensus on the mechanism of its origin at this time.

Pulmonary flow murmur: This is a harsh quality, grade 2-3, medium-high pitched, ejection systolic murmur. This murmur is best heard with the diaphragm of a stethoscope. It is thought to originate from the right ventricular outflow. It is best heard at the left upper sternal border over the “pulmonary area” and it often radiates to the back and/or axillae. This murmur can be very prominent in high output states and in patients with pectus excavatum deformity of the chest. Pulmonary flow murmur can be differentiated from pulmonary valve stenosis murmur by its quality and absence of pulmonary valve click. Pulmonary flow murmur is also very responsive to change in flow dynamics. The intensity increases with inspiration and lying down due to an increase in venous return, and decreases with standing up and Valsalva maneuver.

Supraclavicular systemic flow murmur: This is a harsh quality, crescendo-decrescendo, medium-high pitched, grade 2-3 murmur heard over the supraclavicular region with radiation to the carotids. This murmur is best heard with the diaphragm of a stethoscope and can be present in any pediatric age group as well as young adults. In contrast to aortic valve stenosis murmur, this is not accompanied by the systolic click of the aortic valve. This murmur is thought to arise from the normal flow of blood from the aorta into head-and-neck vessels [ 12 ]. This murmur is loudest in a supine position and it diminishes in intensity with hyperextension of the neck.

Venous hum: This is a continuous low-pitched murmur best heard over the lower neck, just lateral to the sternocleidomastoid muscles. This originates from the systemic venous return of the superior vena cava. Due to its low-pitch, it is best heard with the bell of a stethoscope. This murmur is very sensitive to position and disappears with making the child look down or to the side. It is most prominent in the sitting position and diminishes or disappears on lying down. This can be easily differentiated by patent ductus arteriosus murmur which is harsh machinery in character and does not change or disappear with changing neck position.

Assess beyond the murmur

A pediatric cardiovascular examination should not primarily focus and end with listening for the presence or absence of heart murmur. A detailed birth history, past medical history, growth chart assessment, detailed family history, and head-to-toe assessment are essential components that help with making a competent clinical diagnosis [ 3 ]. Many of the complex and serious cardiovascular abnormalities may not present with a heart murmur. A cardiovascular exam should include inspection and palpitation of precordium, assessment of peripheral pulses, assessment of perfusion, abdominal organomegaly, etc. in addition to auditory assessment. Lastly, auscultation should include assessment of heart sounds (S1 and S2) first before focusing on the heart murmur. In the pediatric population, the sequence of assessment may need to be modified based on the patient’s age and level of cooperativeness.

Impact beyond the patient

Referral of a child to a cardiologist for evaluation of innocent heart murmur has a significant impact that extends beyond the patient. Diagnosis of heart murmur, even if innocent, is associated with significant healthcare resource utilization and parental/caregiver anxiety [ 13 - 17 ]. Appropriate use criteria for initial transthoracic echocardiography were released by the American Academy of Pediatrics, American Heart Association, American Society of Echocardiography, Heart Rhythm Society, etc. in the year 2014 which clearly notes transthoracic echocardiogram for presumed innocent heart murmur as “rarely appropriate” indication [ 18 ]. Despite published guidelines, there is a significant utilization of transthoracic echocardiography as well as recurrent cardiology visits for innocent heart murmur [ 13 - 14 ]. Giuffre et al. and Bardsen et al. looked at parental anxiety after a diagnosis of childhood heart murmur [ 15 , 17 ]. Both studies reported significantly higher anxiety level in parents after the initial diagnosis. Even when the parents were given a handout regarding innocent heart murmur prior to the evaluation by a cardiologist, it did not decrease the anxiety significantly. Parental anxiety was significantly relieved after evaluation and counselling by a pediatric cardiologist. It should be further explained to parents that child may never “outgrow” the murmur and murmur may only be intermittently audible.

Role of auditory training and computer-aided auscultation devices

Auscultation skill is the cornerstone of competent cardiovascular examination. Evaluation of heart sounds and heart murmur plays an important role in making an accurate cardiac diagnosis and dictate the management plan. But with modernization and availability of various “gadgets” in medicine, unfortunately, this is becoming a neglected skill [ 19 ]. Suboptimal performance by practicing physicians and medical trainees has been well documented in various studies [ 20 - 21 ]. Finley et al. showed in their study that well planned auditory training program rapidly teaches students to distinguish between innocent and pathological murmur with at least 90% accuracy [ 22 ]. They also further show that the skill level does decline over time but can be remastered with repeat training. Repetition and practice are the key factors to master auscultation skills [ 23 ].

There are a few commercially available softwares in the market designed to aid with determining pathological nature of a heart murmur [ 24 ]. These softwares have improved significantly over time but they still lag high specificity and sensitivity for making an accurate diagnosis consistently. Additionally, the patient’s age and heart rate significantly affect its sensitivity and specificity. There softwares put forward a very promising premise for use by frontline providers to differentiate innocent vs pathological murmur with high certainty. But for the time being they are not ready for routine use in author’s opinion.

Conclusions

In conclusion, innocent heart murmur is a benign/functional sound produced by a flow of blood through the heart. It remains a significant source of diagnostic uncertainty among primary care physicians and it is still the most common reason for a referral to a pediatric cardiologist. Despite being an “innocent finding”, it still remains a significant source of health care resource utilization and parental anxiety.

Heart murmurs

Heart murmurs are sounds — such as whooshing or swishing — made by rapid, choppy (turbulent) blood flow through the heart. The sounds can be heard with a device called a stethoscope. A typical heartbeat makes two sounds like "lubb-dupp" (sometimes described as "lub-DUP") when the heart valves are closing.

Heart murmurs can be present at birth (congenital) or develop later in life (acquired).

Some heart murmurs are harmless (innocent). An innocent heart murmur is not a sign of heart disease and doesn't need treatment. Other heart murmurs may be a sign of a serious heart condition. Tests are needed to check the heart and heart valves. Heart murmur treatment depends on the cause.

Harmless (innocent) heart murmurs usually don't cause any other symptoms.

Symptoms of worrisome heart murmurs depend on the cause. Heart murmur symptoms may include:

Blue or gray fingernails or lips
Cough that doesn't go away
Swollen liver
Swollen neck veins
Heavy sweating with little or no activity
In infants, poor appetite and lack of growth

When to see a doctor

Most heart murmurs aren't serious. If you're concerned about a heart murmur, make an appointment to see your primary care provider. Your provider can tell you if follow-up care is needed.

A heart murmur is caused by rapid, choppy (turbulent) blood flow through the heart. A heart murmur may happen:

When the heart is filling with blood (diastolic murmur)
When the heart is emptying (systolic murmur)
Throughout the heartbeat (continuous murmur)

Harmless (innocent) heart murmurs

A person with an innocent murmur usually has a typical heart. Innocent heart murmurs are common in newborns and children.

Things that might change blood flow and cause an innocent heart murmur include:

Lack of healthy red blood cells that carry oxygen to body tissues (anemia)
Overactive thyroid (hyperthyroidism)

Innocent heart murmurs may go away over time. Sometimes, heart murmurs continue for life without causing serious health problems.

Worrisome heart murmurs

In children, worrisome murmurs are usually due to a problem with the heart's structure that's present at birth (congenital heart defect).

Congenital causes of worrisome heart murmurs include:

Holes in the heart. A hole may form before birth in the wall between the upper or lower heart chambers, or both. Examples are atrial septal defect and ventricular septal defect.
Cardiac shunts. A heart structure problem present before birth (congenital heart defect) causes irregular blood flow between the heart chambers or blood vessels.

In adults, worrisome heart murmurs are usually due to heart valve problems that develop later in life (acquired heart valve disease). Things that can damage the heart valves include:

Calcium deposits. Calcium deposits can cause a heart valve — such as the mitral valve or the aortic valve — to become stiff and narrow (valve stenosis). A calcified valve also may not close completely, letting blood flow backward. The changes in blood flow create a murmur. When calcium affects the heart valves, it's called calcific or degenerative valve disease.
Infection of the inner heart lining and valves (endocarditis). Bacteria or other germs cause this infection. The germs spread through the blood to the heart from another part of the body, such as the mouth. A murmur may occur if the infection is untreated and affects the heart valves.
Rheumatic fever. This is a serious complication of strep throat. It's rare in the United States. Rheumatic fever may occur in those who don't get or complete treatment for strep throat. When rheumatic fever affects the heart valves, it's called rheumatic heart disease. A heart valve problem can cause a murmur.

Things that increase the risk of heart murmurs in babies includes:

Family history of heart problems linked to murmurs
Uncontrolled diabetes in the mother during pregnancy
German measles (rubella) in the mother during pregnancy
Use of certain medications, alcohol or illegal drugs by the mother during pregnancy

Some medical conditions can increase the risk of heart murmurs, including:

A rare cancerous tumor that releases certain chemicals into the bloodstream (carcinoid syndrome)
A weakened heart muscle (cardiomyopathy)
An infection of the lining of the heart (endocarditis)
Blood disorders marked by a high number of certain white cells, called eosinophils (hypereosinophilic syndrome)
Certain autoimmune disorders such as lupus and rheumatoid arthritis
Heart valve disease
High blood pressure in the lungs (pulmonary hypertension)
History of rheumatic fever

There's no known prevention for heart murmurs. But healthy lifestyle changes can improve heart health and prevent some conditions linked to murmurs in adults. Many childhood heart murmurs go away on their own as children grow.

An encounter with wild animals in Myrtle Beach is more likely than one might think, and not just the ones that take place on the Atlantic Ocean. Despite all its neon-lit attractions and modern development of the area, a back-to-nature experience with tigers, leopards, chimpanzees, orangutans, wolves and other endangered species are closer than you think at T.I.G.E.R.S, a local wildlife outfit dedicated to the preservation of these majestic creatures. Featuring two facilities on the Grand Strand, T.I.G.E.R.S. allows visitors to get up close and personal with rare and endangered species. T.I.G.E.R.S. Preservation Station at Barefoot Resort in North Myrtle Beach is a free exhibit featuring live tiger cubs and other creatures where visitors can get a sneak preview of bigger and better things at the larger facility in Myrtle Beach. Located about 17 miles south of the Barefoot Landing building, T.I.G.E.R.S. Wild Encounters Preserve features a large collection of animals with lots of room to roam. Trained handlers bring the animals to the free display area where visitors can pet, feed and take photos with some of the tamer species on hand. Professional photographers and videographers are on hand to capture the magical moments of bonding between man and animal, and they are available for purchase at the end of the tour. As one of the few breedng facilities in North America, T.I.G.E.R.S. often has young cubs on site, and also breeds the rare liger (half lion, half tiger). Guests can hold a tiger cub in your arms and feed it from a baby bottle, or watch as handlers run larger animals through the paces. Many of the animals from T.I.G.E.R.S. have been used in Hollywood movies, such as the “Ace Ventura” series, and they are trained on-site by T.I.G.E.R.S, which stands for “The Institute of Greatly Endangered and Rare Species.” Tours are available on Tuesdays, Thursdays and Saturdays beginning at 10 a.m. Children 6 and under are not allowed to take part in the tour due to safety regulations. Fantastic Experience!

My wife and I did the half day package on our honey moon and it was incredible. If you are an animal lover this is something that has to go on your bucket list because you are supporting a great cause and an experience that you won't forget. We fed Bubbles (the elephant), held a baby orangutan, tiger cub, wolf pups, watched the tigers play in the water and one chase down a chunk of meat. The staff were very knowledgeable and professional and it was a very interactive program.

This was an absolutely incredible experience and worth every penny. We did the daytime safari and were there from 9-130, the first hour was a little bit of sitting around and getting to know the place, but then it is constant action at a reasonable pace. They fed us a few times and kept us entertained. The Ligers and Lynx and Wolves and Monkeys and elephant were incredible and the staff was very knowledgeable and caring. If you are in the area, book your safari! It was truly a once in a lifetime opportunity and I will recommend it to everyone!

If you're considering going to Myrtle Beach Safari. Do it!!!! I highly recommend. I fulfilled one of my biggest dreams, to hold a monkey and chimpanzee. The whole experience is incredible. You learn so much. You get to touch almost everything. I loved all of the people working there. You can see how much they love the animals.. You get to take your time with the animals. Today was seriously one of the best days of my life!. Everyone needs to experience this once in their life. Everything that's included in the Safari is amazing.. they took 300 of the most beautiful pictures and that are included in the price. Well worth it.. i can not tell you enough how much I loved this!

This was our first time and it was amazing! The staff was very friendly and always gave clear instructions. They give you drinks and snacks throughout the tour, then do pizza and chips and drinks towards the end. We did the daytime safari from 9am-1:30pm. It is expensive but you get a lot of interactions with the animals and lots to see. It down poured for about 15 minutes while we were there, meeting and learning about Bubbles the elephant. They give you umbrellas if you choose to stay out from the covering, but there is a covered area if you choose not to get rained on at all. This is definitely something that needs to be added to your bucket list!

Absolutely amazing time here! We did the night tour, and after a slow start getting everyone checked it, the tour was fantastic. You can tell the animals are truly loved here and you get to interact with so many. We can't wait to come back and do the day tour!

My family loved there entire day at the Safari and then the highlight was hanging with Bubbles and his beautiful family at there home and taking a dip in the intercostal's waterway, off the hook Awesome memories made that will never be forgotten

I waited for over a year to visit here. It did nit disappoint! The day Safari was amazing. The brought out tons of animals and gave you ample time with them. They also gave you snacks through. It was about 3 hours of pure pleasure and excitement. I have been waiting to see Angada for over a year and he was so cool and fun!! Thank you Moksha for sharing him!! Speaking of Moksha, she was a delight! Super friendly and always smiling! The night tour was fun. There was a campfire with the 2 older chimps but you didn't get to interact with. If you go to visit, which I highly recommend, plan for the day tour. You can also get a photo package which gives you a ton of great photos!!

Amazing! I wasn't sure what to expect. I thought it may be a group thing where there was no real interaction with the animals, but I was definitely wrong. I was really pleased with the amount of interaction with the animals. I won't give a play-by-play of the experience because you need to go experience it for yourself, but I will say you will not be disappointed. Yes the ticket price seems high but I feel like I really got an opportunity to do something I would never be able to do on a multi-thousand safari. This will definitely be an annual birthday tradition. There are snacks and lunch throughout. The grounds are amazing. Don't be thrown off by the dung smell when you arrive...it's just bubbles fertilizing the fields. Only downside: There may be a couple parts of the safari that would be hard for someone to completely participate in if you have trouble sitting on the ground and there are a small flight of stairs to climb (less than 10 steps). Not sure if there was a ramp. The staff is very kind and a lot of fun. They take a ton of pictures of you and those are included in the ticket price - one note is the photos are of your group so if you also want individual pics there is an additional fee. We are doing the night safari in October 2023! Can't wait!

We were most fortunate to be introduced to the wonder that is Myrtle Beach Safari. We enjoyed both the Tiger Palace Overnight Stay and then the Chimpanzee Paint and Play the next day. Doc, Kody, Tila, Sarah, Rob, Alice and Rikki are not only wonderful hosts, they encourage questions and want guests to not only enjoy but also learn. All of the animals are part of an extended family. Our interactions with the animals, from feeding magnificent tigers, having a fun breakfast with Bubbles (elephant) to playing with chimpanzees, lemurs and wolf pups were, frankly, emotional. There was much more interaction beyond my short description above. It's just too much for words. In 24 hours our understanding of what the human/animal bond can be was absolutely expanded. When you look into the eyes of a chimpanzee you see understanding and emotion; it's truly moving. If you have the opportunity to visit this magical piece of the world do so.

So I have been trying to get in touch with this place for a month. I finally get through and this lady answers and I asked her if I could ask some questions. She did not want to be bothered. This happened at 8:15 on 7/24/2023. So who we was working there or answering phones were rude as anything. Then I ask her is this a bad time and she said, what can I help you with in a very rude manner. So I was asking questions and then she let out a si with her voice like I was bothering her again. My son has been asking me to go to this place for 3 months since we booked Myrtle beach from august 12 to the 19th. Now he will be heartbroken because of the way this lady treated me. How could anyone expect me to pay $389 per person, when they get treated like I just was. Absolutely terrible. Maybe someone can explain to me why this happens.

A wonderful experience for everyone! When I was gifted this experience from a family member I was so excited to go as I have heard of this safari tour before! Not only did I get to meet some amazing animals eye to eye, but I also got the chance to interact with them in a way that was so special and memorable! The staff - incredible! They're top notch and you could tell that these animals are their entire lives! A very family run preserve it's no secret that these animals have all the love they could possibly need and more! The way they love and interact with each other and the animals is worth seeing as well! It's just beautiful! Thank you! I've been on many animal adventures in the past, but nothing has been quite like this! I want to gush about all of the things that happened on the tour, but that would just not be fair to anyone who will be experiencing this safari for the first time! It's a must see/must do, and worth every dollar! (Plus not to mention all the things they do for animals all over the world!) Let them guide you through an exciting adventure that is sure to fill all your animal loving needs!

The most amazing experience my husband and I had ever done...it was the highlight of our trip to Myrtle Beach area...tour cost goes toward TIGERS Preservation in helping with endangered species. ..sad to say only about 3,000 tigers left in the wild...highly recommended. ..hands-on, up close intervention with AAA+ staff

This is our second time here and it was as fabulous as the first time...even with the rainfall. It's nice to see theses beautiful creatures so well cared for and in a peaceful, private surrounding. There is always something new to learn about the animals and their challanges in the wild. I'm now very conscious about the products I buy so that it will not contain Palm Oil. As the use of palm oil increases in products, the remaining habitats for these wonderful creatures are decreasing around the world from harvesting that ingredient.

For the third time in 2 years the staff at T.I.G.E.R.S Preserve has amazed us. It is worth every penny and more. My daughter and I decided to give a tour to her Grandmother for Christmas. She has heard about it for almost a year now and has now experienced everything we talk about. She already said she will be back again this summer. You will never understand how amazing it is until you experience it. To be able to hold tigers and lions and chimps and gibbons and Wolf cubs and ligers and a serval and touch an elephant where else can you do this! Absolutely AMAZING! Barefoot Landing is a popular location for shopping, dining and various attractions. It is also the home of a free wildlife and living tiger museum in the T.I.G.E.R.S Preservation Station . Not only are visitors encouraged to enjoy the museum at no cost, but they also get to see some of the world’s most rare tigers. The Bengal tiger, Siberian tiger and Royal White Bengal tiger can all be viewed in their outdoor environment. They also have a Golden Tabby tiger which is the most rare tiger in the world.

Tiger trainers are available to answer questions and provide fascinating information about these beautiful feline creatures. Apes are also among those animals at the Preservation Station. As an opportunity to allow visitors to help support their efforts of conserving and breeding tigers, a photographer is always on hand to take pictures of visitors and their families or friends snuggling up face to face with a baby tiger or an ape. The cost of $59 is pretty pricey for one 8 x 10 shot, but when you consider that you actually get up close and hold one of these wondrous creatures, it’s hard to put a price on it. Guests are happy to know that the proceeds go to help the preservation of the tigers. Be sure to check for coupons as you’re visiting the Grand Strand for discounts. They can be found in the Monster and Sunny Day coupon books.

There is another option available that will greatly enhance your experience at T.I.G.E.R.S. In addition to the Preservation Station at Barefoot Landing, they also have a 50-acre preserve in Myrtle Beach that offers the Wild Encounters Tour. Those people who take part in this preserve are almost speechless as they describe their time spent getting to know the tigers, leopards, orangutan, wolves and chimpanzees.

This adventure will be a once in a life time event for most who choose to go. The price is an extravagant $199 per person. But those who report back say that it was more than worth the cost. Photo and video opportunities are also available at the preserve. As you go through the tour, professional photographers will take studio quality pictures and videos of your experience. If you choose to purchase the photo package, they will take $40 off the cost of your tour and of every person in your party. This will lower the price of each person in your group to $159. You will still pay for the pictures, but at least part of it will come off the tour cost. The local coupon books offer $20 off the price for each person in your group. Be sure to use those to make the price a bit more affordable.

One 8 x 10 picture per group will come with the price of your tour. What you would actually purchase would be the Photo CD at a cost of $125 if purchased in advance. The CD is a professional portfolio that includes pictures of each individual in the group with the animals, group pictures with the animals, candid shots of your group throughout the tour and pictures of the animals. So in the cost of the CD you will receive hundreds of photos, along with the right to reproduce them for printing. There will also be a Video DVD of your tour that costs $125, but if you purchase this along with the Photo CD, the cost is only an additional $25. So for $150 you will get both the CD and DVD. These prices only apply if you purchase them in advance of the day of your tour. The price goes up if purchased on the tour day. Yes, all the photos, videos and price of admission are expensive, but do remember that they use this as a means of raising money to support their endangered species friends.

Children must be at least 6 years old to go on the tour. You will not be able to bring cameras or cell phones. Be sure to leave jewelry such as bracelets and chains behind in a safe place. Also, do not wear white clothes, dresses or skirts, and do wear closed toe shoes.

Baseball develops some of its best elements in the Minor Leagues, from the talent of players, to ballpark promotions to rule changes. That also extends to food. Of course, you can get all your favorites -- Cracker Jacks, hot dogs, cotton candy, beer, cola and all the rest -- at any Minor League park, but almost all of them also offer regional specialties, experimental festive fare or a combination of both.

The IronPigs, as you might have guessed, have a thing for bacon. They also have a thing for, well, just about every kind of food. So robust is the Lehigh Valley concessions scene that the team maintains a standalone website dedicated entirely to in-park offerings. One of several new additions this year is the Sweet Pig Burger, which gets the “sweet pig” part of its moniker from the black peppercorn candied bacon it’s topped with. It also includes candied jalapeños and is served on a brioche bun.

TITANIA for charter

Delivered by Lurssen in 2006 with stern extension in 2012 and a full interior refit in 2018, TITANIA’s exterior was penned by the renowned designer Espen Øino who created a timeless, sophisticated silhouette. Sumptuous accommodation is arranged across seven cabins, including two expansive master suites, a VIP cabin and four generous doubles, all with beautifully appointed marble bathrooms and the latest entertainment systems, ensuring all guests enjoy unparalleled comfort and luxury. The main deck master suite features a study, walk-in wardrobe, jacuzzi bathroom and private massage area.

The sun deck offers plenty of sun loungers and a stunning enclosed glass veranda accommodates a climate-controlled gymnasium with top of the range equipment. A fitness instructor is available on board for personal training sessions.

Well-being is a high priority on TITANIA and the resident massage/beauty therapist and hairdresser offer a range of treatments in the dedicated spa room on the upper deck, which leads out to the spectacular 4.5m long swimming pool/jacuzzi with swim-up bar and fountain feature. You can be pampered with a lime and ginger salt scrub or hot stone massage, before cooling off in the inviting pool.

The sea level beach club provides a sauna, bar and relaxing seating area with easy access to the water and the vast inventory of tenders and toys. The yacht is an accredited RYA training centre, with two qualified jetski instructors who can help guests pass their licenses. At night the beach club converts into a party zone with disco lights and state-of-the-art sound system. A complete aqua park can be deployed from the beach club to provide a safe splash zone, inflatable slide, trampoline and climbing frame, providing hours of fun for the whole family. The yacht also has one of largest superyacht slides ever built running from the dizzy heights of the sun deck.
